Learning Extension Center Volunteer Application

It is our desire to reduce the risk of abuse within our church ministries.  We believe this information is necessary to protect our children, young people, and volunteers, and to effectively place our volunteers in ministry positions.  Thank you in advance for your partnership.

Are you willing to submit to a criminal background check? (Please note that failing to submit to a criminal background check will result in the applicant not being offered a volunteer position)

*If you are uncomfortable sharing your Social Security Number here please call Kari Lashley at 614-885-3929.

In order to provide a safe and secure environment for the student’s that access our building, we believe it is necessary to ask the following questions as part of our application process. All information will be kept in strict confidence by the LEC Leadership Team.  It will not be disclosed by the church unless required by law.  Answering “yes” to any of the following questions may not necessarily prevent you from volunteering with the church. Thank you in advance for your understanding.

Have you ever been convicted or found guilty of a criminal offense for which a pardon has not been granted? (Note: This does not include minor traffic violations.)

Have you ever been expelled from or had your employment terminated by any organization or employer for assault or violence against any person, or for assault, violence, or impropriety with children, young people, or vulnerable persons?

I hereby give the church permission to verify the information provided by me in this LEC Volunteer Application Form.

I grant my permission for the church to perform a criminal background check on me.  If you have a complete copy of a current criminal background check (within the past 12 months), it may be submitted with this paperwork.

I further grant the church permission to perform an online search on me, and to review and consider any information found about me on the internet.

I understand that if the church approves my volunteer application and later determines, in its discretion, at any time, that I am not suitable for volunteer service in the church, or for the volunteer position for which I am applying, the church may terminate my volunteer service or volunteer position for any reason without advanced notice.  I will, at all times, cooperate fully with the staff of the church in the fulfillment of my duties, and will keep confidential all information I encounter in my role as a volunteer.

I hereby acknowledge that, to the best of my knowledge, the information contained in this form is true and correct.

NOTE: There is a mandatory volunteer training on Wednesday, September 9th from 6:30 – 8:00 PM in the Fellowship Hall.
